Subacute Rehabilitation

      The Recovery Care Program at Harbour Health and Hawthorn Health provides several specialty areas for short-term rehabilitation.  The program also focuses on patients that are deconditioned due to hospitalization with various diagnoses.

      Both facilities are committed to providing quality care to patients in need of clinical and rehabilitative services provided at Harbour Health and Hawthorn Health.  The program’s goal is to return the patient to his or her maximum level of functioning.  The plan is interdisciplinary and focused on the needs and goals of the individual patient.

• Staffs of the facilities have experience with caring for the following types of patients:
• Orthopedic (fractures, total knee replacements, etc.)
• Stroke
• Cardiac
• Palliative (radiation)
• Pain control

      It is the goal of the interdisciplinary team to allow the patient to recover and reach his or her previous level of functioning before returning safely to home.
